New forms comming up

Currently, CIBMTR and NMDP are creating a new single series of Report Forms for submitting data to the 2 organizations. The expected release of the new Forms is later in 2006. The Registration Forms are being updated by CIBMTR and EBMTR, also with an anticipated 2006 release.

Q9 – took the “NOS” off of “American Indian, NOS”
Q185 – added “Oral and IV” check-box
Q788 – changed the variable number for “Other, specify”

Changes in 8/03 version:
Q645 – changed “post-DCI” to “post-HSCT”
Q472 – added variable 4 “Chronic GVHD flair (symptoms reactivate within 30 days of drug tapering or discontinuation)”

Changes in 5/03 version:
